| Employee Attitude Survey |

| Purpose |
| The Employee Attitude Survey is administered to ascertain the morale and
motivation of employees; to, in effect, take a temperature reading of the
organization so that a proper prescription for improvement can result. |

| Procedure |
|
Step One - Design of the Questionnaire
In addition to the core questions, the special needs and/or circumstances
of the organization are taken into consideration in order to design additional
questions.

Step Two - Collection of the Data
Data is collected on-site through use of the questionnaire, usually
over the course of one day per location. Special arrangements are made for
absentees or in cases where on-site data collection is not feasible. |

Step Three - Presentation of the Survey Results to Management and Action
Plan Design
Management receives feedback first, usually within two to three weeks
of questionnaire collection. The process of Action Plan development is begun
in preparation for the presentation of Survey results to all employees. |

Step Four - Presentation of the Survey Results to all Employees in
Conjunction with Management's Action Plan
The presentation to all employees involves the viewing of the answers
of key statistical questions, the reading of representative comments from
the participants, and a review of the recommendations, followed by management's
response through a summary of the newly developed Action Plan. |

Step Five - Monitoring of the Action Plan
Monthly pulse taking phone calls or visits are made to check on the
progress of the Action Plan, and a future Survey is planned - four to 12
months down the road - for comparison purposes.Requirements Management agrees
to absolute employee confidentiality with no negative repercussions for
any employee feedback. Management agrees to develop and implement an Action
Plan based on the results of the Survey. Management agrees to make the complete
Survey results available to all employees by making them accessible through
H.R. and/or the management team.
